How you can make a referral to A4MHD
To make a referral to us you can call us at the office, write to us or complete the referral form (click on the yellow block above), which you can post or fax to us. We always encourage people to refer themselves but accept referrals on behalf of people too, from family, friends and professionals alike. If you’d like to discuss a possible referral please call the office and speak to someone.
Phone the office on 0113 247 0449 between the hours of 9.00am and 5.00pm on weekdays (Monday to Friday), or if calling out of hours, leave a message on our answermachine.
If you have access to the internet you can also email the office at office@a4mhd.org.uk or use the online referral form to contact us,
or you can download and complete the referral form as previously indicated.
